  • Applicants must be businesses located in the City of Mesa. Verify your location here: (EXPLORE MESA).Tip: You can then copy the address returned into the application.
  • Business must have suffered negative financial impacts or business interruptions due to required business closures or limited service due to the COVID-19 pandemic.
  • Provide a signed W-9. You can print or fill out online then download a W-9 form here.

NOTE: You must upload documentation that matches the business address listed on the application.
